Your Rights as a Patient
As a patient of Alvin-Manvel Family Clinic, you have the right to:
- Receive respectful, dignified care regardless of race, color, religion, sex, national origin, age, disability, sexual orientation, gender identity, or source of payment
- Be fully informed about your diagnosis, treatment options, and prognosis in terms you can understand
- Participate in decisions about your care and treatment plan
- Refuse treatment or procedures, and to be informed of the consequences of that refusal
- Privacy and confidentiality of your medical information, in accordance with HIPAA and Texas law
- Access your medical records and request amendments (see our HIPAA Notice)
- Know the identities and professional credentials of the providers treating you
- Receive a clear explanation of your bill and ask questions about charges
- Receive care without unnecessary delay when medically appropriate
- Be informed of our policies, including those affecting your care
- File a complaint or grievance without fear of retaliation or discrimination
- Receive language assistance services if English is not your primary language
- Request an interpreter or communication assistance if you have a disability
Your Responsibilities as a Patient
In return, we ask that you:
- Provide accurate and complete information about your health history, medications, allergies, and symptoms
- Inform your provider if you do not understand your diagnosis or treatment plan
- Follow the treatment plan agreed upon with your provider, or communicate your concerns if you cannot
- Keep your appointments and notify us in advance if you need to cancel or reschedule
- Treat our staff and other patients with respect
- Provide accurate insurance information and pay your portion of the bill promptly
- Accept responsibility for outcomes if you refuse recommended treatment
How to File a Complaint
If you have concerns about the quality of care you received, a billing issue, or any aspect of your experience at AMFC, we want to hear from you. Our goal is to resolve concerns promptly and fairly.
Step 1: Contact Our Office
Speak directly with your provider or a member of our administrative team. Many concerns can be resolved quickly when raised directly. You may also submit your concern in writing.

Step 2: File a Complaint with External Organizations
